Be sure to bring two forms of ID, one with picture (Drivers License or Passport).  You will fill out an FCC form 605 - your amateur radio license application. It asks for contact information (name address, phone/fax number, email, birth date etc.) and Social Security Number. Only your name and address are part of the public record. The FCC maintains the rest for identification purposes. The phone numbers and email are optional but will help if there's a problem processing your application. The address and SSN are NOT optional.

The FCC also offers the ability to use a Federal Registration Number (FRN) rather than an SSN. You can apply for this FRN at the FCC website.
